If you are participating in a Washoe County School District approved internship, you will need to complete the following:

• Work-Based Learning Permission & Indemnification Forms (3 forms total to complete & return)
  • Work-Based Learning Parent/Guardian Consent
  • Student Private Transportation Permission
  • Parent/Guardian Waiver of Liability
• Washoe County School District Work-Based Learning Training Agreement (1 form to complete & return)
• Workplace Safety Training & Quiz (read/watch 5 training articles/videos & answer comprehension questions)
