Step 2: On the Insurance Benefit Enrollment Form, you will have the option to select between either the PPO or HDHP. If you choose to enroll in the HDHP, you will also need to complete the HDHP HSA Enrollment Form. Please note, regardless of whether you select the PPO or HDHP, if you are looking to add dependents, you will need to provide additional legal documentation. For example, if you are looking to add your children to your plan, you will need to provide a copy of their birth certificates to complete your enrollment.
