• We understand that changes happen! Below are the most common forms that you might need as changes in your life occur. Please reach out to our HR/Benefits office at 775-348-0321 or benefits@washoeschools.net if you have any questions on the following.

    If you complete any of the other following forms, please email a copy to benefits@washoeschools.net and someone from our Benefits Department will review the request and be in contact with you. 

    If you have experienced a qualifying event and need to make changes to your benefits outside of open enrollment, please complete the following form: Benefits Enrollment Change Form

    If you have moved or changed your name, please complete the following form: Change of Name and Address Form

    If you need to update your beneficiaries for your life insurance, please complete the following form: Life Insurance Beneficiary Form

    Specific requirements need to be met for the Waiver of Contributions, please reach out to the Benefits Department for more details: Request of Waiver of Insurance Benefits Form
