Medications at School
Parents/guardians should make every effort to avoid the necessity of medicating students during the school day and, whenever possible, parents/guardians should develop medication schedules that provide for the administration of medication before or after school hours.
When there is no reasonable alternative and students are required to take prescription or non-prescription medication during school hours with assistance from school personnel, there must be on file in the school health office a “Consent & Request for Medication Assistance During School Hours” form. If prescription medication is labeled by the pharmacy, the parent/guardian must sign the form; if the medication does not have a pharmacy prescription label (OTC, etc.), the form must be signed by the parent/guardian and an authorized healthcare provider. Please note: there are specialized forms required for students requiring anaphylaxis medication or insulin.
