BYU Independent Study Contract
You must get counselor approval PRIOR to enrolling in a BYU course.

Peer Tutor Contract
It is the student's responsibility to find the teacher they would like to be a peer tutor for. Once you obtain a teacher's signature, return the completed form to your counselor.

Student Aide Form
It is the student's responsibility to find the teacher they would like to be an aide for. Once you obtain a teacher's signature, return the completed form to your counselor.
Please note the GPA requirement to be a teacher's aide is a 3.0 and the GPA requirement to be a department aide is a 3.25.

Work Experience Contract
The contract needs to be completed and returned to your counselor with a CURRENT paystub.
