Requirements:

Students must complete all 2021-2022 athletic packets.
- Athletic Participation Physical Forms
- Quick form for students who have already participated in a sport this year (available from the athletic secretary in the main office)
Any/all clearance forms, physical forms, health questionnaires, and pre-participation history forms must be printed, completed, signed by the student, parent and/or physician, and returned to the athletic secretary in the main office.
Students must be registered on Register My Athlete (see below).
- Students who have already created a Register My Athlete account: You must log into your account and change the year to 2021-2022 and add your sport(s) for this year.
- New athletes: You must first create your account, then follow the directions on the Register My Athlete website.
- If you are transferring from another high school and already have a Register My Athlete account, it is possible to merge your accounts. Directions on how to do this will be posted here as soon as they are available.
