Leave of Absences

Leaves of Absences are required if you are going to be using more than 5 sick days. Leave of Absences are approved/denied based on each Negotiated Agreement. 

The Benefits Department has been working on streamlining the leave of absence process to make requesting time off easier and more efficient.  As part of this effort, we’re introducing a new automated form that ensures all necessary parties are notified when a leave request is submitted. 

If you are taking more than 5 consecutive days off, please submit a leave of absence request form using the link below.
