Benefits Enrollment Information

If you’ve secured a position working 27.5 hours or more per week, you are eligible for our comprehensive benefits package. We offer:

  • Medical, Dental, and Vision coverage
  • Life Insurance
  • Voluntary Products (additional coverage options)

Step 2: Complete Your Enrollment Forms

  • Once you have reviewed all of the information and are ready to enroll in district insurance benefits click here to complete an adobe enrollment packet. Once the enrollment packet is submitted you will receive a notification via email that your packet has been received by the benefits office.
  • You will also receive a confirmation email from your Benefit Specialist once the packet has been successfully processed and you are enrolled in benefits.       

 On the Insurance Benefit Enrollment Form, you’ll choose between:

  • PPO Plan
  • HDHP Plan
    (If you select HDHP, you must also complete the HDHP HSA Enrollment Form.)

Required Documents to bring to WCSD:

  • If enrolling dependents in benefits, please submit the following to Benefits:
    • Marriage Certificate
    • Declaration of Domestic Partner Certificate
    • Birth Certificate 
    • Court Ordered documentation
    • Front page of Federal Tax Return

If You Decline Coverage

If you choose not to enroll in WCSD health insurance, please complete the Benefits Refusal Form acknowledging your decision.

Important Information:

  • Benefits will be effective on your first day of employment if all documents are received by our Benefits Office beforehand. 
  • Once you are enrolled in benefits you can access a digital Anthem card on the Sydney health app or at Anthem.com. Click here to register on Anthem.com or click here to download the Sydney Health app for access to your benefits, ID Card, pharmacy info and more. 
  • If these forms are not completed and returned to the Benefits Office until after the 30 days of your hire date:  
    • You will not be enrolled until the 1st of the month following receipt of your completed paperwork in the Benefits Office.
    • You will not be able to add dependents until the next OPEN ENROLLMENT UNLESS YOU HAVE A QUALIFYING EVENT. Changes made during open enrollment take effect January 1st of the next calendar year.
