ADA Accommodations
What is ADA?
To be provided a reasonable accommodation, you must have a disability as defined under the ADAA. A disability under the ADAA with respect to an individual is a physical or mental impairment that substantially limits one or more major life activities of such individual; a record of such impairment; or being regarded as having such an impairment. “Major Life Activities” includes but is not limited to caring for oneself, performing manual tasks, walking, seeing, hearing, sleeping, standing, lifting, bending, reading, concentrating, thinking, communicating, speaking, breathing, learning and working. It also includes the operation of major bodily functions, including but not limited to, functions of the immune system, normal cell growth, digestive, bowel, bladder, neurological, brain, respiratory, circulatory, endocrine, and reproductive functions.
If you believe you may be eligible for a reasonable accommodation under the ADAA, please complete the enclosed form and include reasonable documentation from a health care provider that your medical condition is a disability. The documentation must be from a qualified expert, such as your physician.
ADA Interactive Process:
1. Employee completes the ADA request: https://app.smartsheet.com/b/form/7a28af2ac71a46e8a87ea7f2ec427a46
2. The Benefits Team will provide more information on the interactive process. We will need a description of the accommodations being requested and doctor's documentation that verifies the disability and outlines accommodation recommendations.
3. Once the documentation is received, the interactive ADA meeting is held with the employee, their supervisor, and a Benefits Team Member to review the accommodations being requested.
4. After the interactive ADA meeting, the Benefits Team Member sends out an approval letter outlining the accommodations that will be implemented.
