Internship Materials for Students & Parents

If you are approved and participating in a Washoe County School District approved Work-Based Learning opportunity, including internships, you will need to complete the following documents:
- Work-Based Learning Permission & Indemnification Forms (3 forms total to complete & return)
- Parent/Guardian Waiver of Liability
- Student Private Transportation Permission
- Work-Based Learning Parent/Guardian Consent
- Washoe County School District Work-Based Learning Training Agreement
- (1 form to complete & return)
- WBL Student Learning Plan-for Individual Experiences
- (1 form to complete & return)
- Workplace Safety Training & Quiz
- (read/watch 5 training articles/videos & answer comprehension questions)
- All forms should be completed & returned to the internship instructor assigned at your school or to Crystal Edwards, the WCSD Internship Coordinator
