Internship Materials for School Sites

Check out our Quick Reference Guide for the criteria for all stakeholders regarding work-based learning experiences.  Most individual internships will need about a semester's lead time to procure and get all paperwork completed and approved. 
 
School Staff- please familiarize yourself with these documents for tracking, approvals, and compliance.
  • Employer Host Vetting FormRequired for every employer host. Completed by the coordinator/school to vet a business or organization before any student is placed there, regardless of WBL experience type. Confirms the site meets NDE safety, harassment-policy, and eligibility criteria, and identifies any employees who need a background check.
  • Washoe County School District Work-Based Learning Training AgreementRequired for CTE Work Experience, and internships over 30 hours. Signed by the student, employer host, and school before the student begins on-site work; outlines each party's expectations and responsibilities. Not required for Job Shadowing.
  • WBL Student Learning Plan  — Required for Internship, CTE Work Experience, and Clinical Experience placements. Completed alongside the Training Agreement; defines the specific academic and workplace skills the student is expected to master during the placement.
  • Student Evaluation of Internship ExperienceRequired at the end of Internship and CTE Work Experience placements. Completed by the student to evaluate their experience and provide feedback used to maintain the employer host's approved status.
  • WCSD Work-Based Learning AgreementRequired for employer hosts to sign per WCSD policy before a student can serve on the job site. Formalizes the overall partnership between WCSD and the business or organization, establishing the terms under which students may be placed there across any WBL experience type. This district-wide agreement must also be approved by WCSD's Purchasing department.
  • Tracking LogRequired for paid or credit-bearing placements (Internship, CTE Work Experience, Supervised Agricultural Experience). Records daily on-site hours and is signed off by both the student and employer host supervisor at the end of the placement.
