Evaluations
Please follow the steps below to request an evaluation through Child Find:
*Please note, only legal guardians or parents can request an evaluation, sign documents and consent to evaluate.
- Contact Child Find to request an evaluation for your child by calling (775)327-0685 or using the Form under "Contact Us" section.
- A Child Find secretary will complete the intake form over the phone with you. This informational form provides a way for you to describe the concerns you have regarding your child. Upon completion of the intake, the secretary will provide you with detailed information on next steps.
- An appointment to evaluate your child will be scheduled.
What to expect:
- Evaluation meeting Based on the concerns you indicate and that you provide written consent, assessments will be conducted with your child. These assessments may include speech and language, developmental skills, fine motor, gross motor, vision and hearing screening, and behavior checklists.
- Eligibility meeting Upon completion of the evaluations, an eligibility meeting will need to occur. At this meeting, the Child Find team, including you, will meet to discuss the results of the evaluation. A determination of whether your child meets the criteria for special education eligibility will be made.
- IEP meeting If your child meets the criteria as a child with a special education disability, an Individualized Education Plan (IEP) will be developed. During this meeting the team, including you, will determine what services will be provided to your child and where those services will take place.
- If your child qualifies for an IEP, you will be provided with the documents and the assigned school location. At this point, once the IEP is complete, you may go to the assigned school and register your child at the school. Since you have already completed online registration, it is a simple process for the school secretary to help you complete the registration. Please make sure that you have immunization records and an original birth certificate as those are required documents for attendance in a school.
