Steps to Complete the Health Assessment

1. Sign into your Personify Health account. If you have not registered CLICK HERE If you have registered Use this link to Log in to Personify Health 
2. Enter your name, gender, and date of birth, and click CONTINUE to verify you are eligible to join.
3. Please read and agree to Personify Health's Data Consent, Privacy Policy and Membership Agreement.
4. Next, enter your information into the fields that follow.
5. Click LET'S GET STARTED!
6. Complete your Health Assessment, which can be found by choosing the Health tab on the Personify Health portal.
